STAR answer examples

Model answers using the Situation-Task-Action-Result framework. Adapt to your own experience.

1Question

Tell me about a time you supported someone through an emotional crisis — what did you do and what was the outcome?

Situation: A service user on a 12-bed acute ward became highly distressed during a night shift following a difficult phone call with a family member. Task: As the support worker on duty, I needed to de-escalate the situation safely and provide immediate emotional support. Action: I calmly approached the individual, used active listening techniques, and stayed with her for 40 minutes while the rest of the ward remained settled. I informed the nurse in charge and documented the incident in full. Result: The service user stabilised without requiring further clinical intervention, and the following morning her care plan was updated to include additional family liaison support. The nurse commended the approach in the handover notes.
2Question

Give an example of when you identified a safeguarding concern and explain the steps you took.

Situation: During a routine welfare check on a rehabilitation ward, I noticed unexplained bruising on a service user who had previously been reluctant to discuss her personal relationships. Task: I needed to act promptly and in line with safeguarding policy without alarming the individual. Action: I documented my observations clearly, reported immediately to the senior nurse on shift, and ensured the service user was not left alone until a full assessment was completed. I also completed the internal safeguarding referral form within the same shift. Result: A formal safeguarding investigation was opened within 24 hours. The service user later disclosed the concern to the clinical team, and appropriate protective measures were put in place. My early identification was noted in the case review.
